Deputy Lead Practice Nurse (with succession to Lead Practice nurse)

An exciting opportunity has arisen for an experienced Practice Nurse to join North Street Medical Care as a Deputy Lead Practice Nurse. This role offers a defined development pathway to the Lead Practice Nurse position, with opportunities to take on wider leadership responsibilities.

You will be supported by an experienced Lead Practice Nurse and a highly supportive management team, enabling your ongoing professional development.

Main duties of the job

Working closely with the Lead Practice Nurse, you will be a senior member of the multidisciplinary team, supporting the effective delivery of high-quality nursing services to our patient population.

Key responsibilities include:

* Delivering and supporting chronic disease management
* Assisting with the development, implementation and review of clinical policies and procedures
* Supporting service improvement and the delivery of preventative healthcare initiatives
* Contributing to the line management and supervision of the HCA team
* Deputising for the Lead Practice Nurse where required

We have a dedicated and friendly multidisciplinary teamincluding GP Partners, Salaried GPs, ANPs, Clinical Pharmacists, PharmacyTechnicians, Allied Health Professionals, a skilled Nursing Team and HCAs,supported by strong administrative and management teams.

As the lead practice within our Primary Care Network (PCN),we work collaboratively with three partner practices and benefit from a widerange of additional roles, including mental health practitioners,physiotherapists, and pharmacy teams.


Job responsibilities

Primary Responsibilities

The following are the core responsibilities of the postholder There may be, on occasion, a requirement to carry out other tasks. Thiswill be dependent upon factors such as workload and staffing levels:

a. Support the Lead Nurse in the leadership of the Nursingand HCA team.

b. Participate in the administrative and professionalresponsibilities of the practice including : Representing the Nurse team atPractice, Business and or external meetings, contribute to Nurse staff trainingand development, ensure Nurse team comply with CQC, NICE, NHSE, national andlocal guidelines, contribute to cost effective achievement of QOF and enhancedservices, promote innovative ideas, be a resource of information, mentorshipand support to others, act as a mentor for new staff and students assistingwith induction and training.

c. Assess, plan, develop, implement, and evaluate treatmentplans that promote health and well-being across the practice population and forspecific patient groups

d. Proactively identify, diagnose, and manage treatmentplans for patients at risk developing and or having a long-term condition,including prescribing/deprescribing for that condition (medical and or social)in line with national and local guidance.

e. For all areas of clinical practice understand theimplications, needs of the patient and signpost appropriately.

f. Contribute to practice targets both local and national,e.g. QOF.

g. Provide adult and childhood immunisation service. FollowPSD/PGD guidelines, UK immunisation schedule.

h. Plan and support leadership team with annual Flu campaignand mass population vaccination programmes.

i. Administer prescribed therapies such as contraceptioninjections/vitamin B12/GNHR/Aranesp/Denosumab and other patient specificinjections, recognising side effects and contraindications.

j. Perform wound management using appropriate(aseptic/clean) technique.

k. Have good understanding of the process of wound healing,knowledge of wound management, potential complications, local and nationalguidance.

l. Be proficient at Doppler assessment, knowledge andunderstanding of the pathophysiology of venous, arterial, and diabetic footulcers.

m. Recognise signs of wound infection, manage appropriatelyand interprets results from microbiology sampling.

n. To oversee the provision of chronic disease clinics,delivering patient care as necessary, referring patients tosecondary/specialist care as required

o. To maintain accurate clinical records in conjunction withextant legislation

p. To ensure clinical codes are used effectively by allmembers of the nursing team

q. To ensure the appropriate use of chronic diseaseregisters

r. Review medication for therapeutic effectiveness.

s. Work with the Lead Nurse and Practice Management team to improve the management of disease within the practice population. Implementclear management plans with patients including appropriate prescribing andreferral.

t. Work to support compliance and adherence to prescribedtreatments by detecting and managing side effects and or adverse reactions.

u. Emphasise the importance of prevention and self care.

v. Refer patients to other services or agencies asappropriate. Promoting communication between all involved in the patient care.

w. To request pathology services and process pathologyresults as required.

x. To implement vaccination programmes for adults andchildren.

y. To support patients in the use of their prescribedmedicines or over-the-counter medicines (within own scope of practice).

z. To liaise with external services/agencies to ensure thepatient is supported appropriately (vulnerable patients etc.).

z.b Take the lead in Infection Control, conduct audits,review policies and procedures, conduct training for all practice staff.

To delegate clinical responsibilities appropriately(ensuring safe practice and the task is within the scope of practice of theindividual)

Secondary Responsibilities

In addition to the primary responsibilities, the Deputy LeadPractice Nurse may be requested to:

a. Develop and review audit protocols and process for thepractice, effectively utilising the audit cycle

b. Develop, review and implement nursing protocols inconjunction with the nurse lead partner and practice management.

c. Support all members of the nursing team, providingguidance when necessary, conducting appraisals and training needs analyses asrequired

d. Participate in local initiatives to enhance servicedelivery and patient care

e. Support and participate in shared learning within thepractice in order to improve patient care

f. Continually review clinical practices, responding tonational policies and initiatives where appropriate

g. Participate in the review of significant and near-missevents applying a structured approach, i.e., root cause analysis (RCA)

i. Lead the planning and implementation of changes withinthe nursing team, enhancing the provision of services to patients

j. Support the practice team with the reviewing andimplementation of practice policies and protocols, ensuring conformance toextant legislation

Qualifications

* Registered Nurse
* Post Graduate Diploma or Degree (Chronic Disease Management)
* Nurse prescriber
* Mentor or Teaching Qualification
* Family Planning Qualification

* Wound Care/removal or sutures and staples
* Smear taker
* ECGs
* Venepuncture
* Requesting pathology tests and processing the results, advising patients accordingly
* Understanding the requirement for PGDs and associated policy
