Overview
Advanced Nurse Practitioner/Advanced Paramedic Practitioner to join the triage team, working up to three days per week (Wednesday to Friday). This role is with Bramblehaies Surgery in Cullompton, Devon.

The registered ANP/ACP will provide a service to patients presenting with minor illness/conditions through our triage service. Applicants must be registered on the equivalent professional register (NMC or HCPC), educated to Masters level in Advanced Practice or equivalent (encompassing the four pillars of clinical practice), hold the digital badge and be an Independent Prescriber. Main duties
* Work as part of a multi-disciplinary team (MDT) within Bramblehaies Surgery and as part of the wider PCN clinical team.
* Assess and triage patients including same-day triage and, as appropriate, provide definitive treatment (including prescribing medications, following policy, patient group directives, national NICE and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
* Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.
* Use complex decision making to inform the diagnosis, investigation, and management of episodes of care within a broad scope of practice.
* Adopt a personalised and population-centred care approach to enable shared decision making with the presenting person.
* Be able to perform specialist health checks and reviews within their scope of practice and in line with local and national guidance.
* Support the delivery of anticipatory care plans and provide an alternative model to urgent and same-day GP home visits for the Practice/PCN.
* Complete relevant training to provide multi-professional clinical practice and CPD supervision to other roles within primary care (e.g., Practice Nurses and personalised care roles).
* Advise patients on general healthcare and promote self-management where appropriate, including signposting to the PCN's social prescribing service and other community or voluntary services.
* Communicate effectively across organisations to deliver a person-centred service; participate in meetings as required.
* Maintain accurate and contemporaneous health records and ensure documentation is completed appropriately for patient care and registration with the practice.
* Communicate with patients and, where appropriate, family members and carers about health needs, results, findings, and treatment choices.

Bramblehaies Surgery is a semi-rural practice in Devon serving 7,500 patients, with a CQC rating of Good. Our team includes GP partners, salaried GPs, a Nurse Practitioner, Practice Manager, and a range of clinical and admin staff. We are a member of Culm Valley Primary Care Network and supported with Social Prescribers and other community services. Our clinical system is SystmOne. We are a training and visa-sponsoring practice hosting GP Registrars and medical students.
