Health and Wellbeing Practitioner

Are you passionate about supporting people to make positive life changes? Join our dedicated team as a Health and Wellbeing Practitioner, working 17.5 hours per week within an integrated healthcare framework to support adults affected by drug and alcohol use.
About the Role
You’ll deliver client-centred treatment and recovery support for offenders entering prison who are identified as having substance misuse issues. Using both harm reduction and abstinence-based approaches, you’ll help clients achieve meaningful progress towards a healthier, more stable future.
Your day-to-day will include:
* Managing a caseload and delivering tailored 1:1 and group sessions
* Providing harm reduction advice and structured treatment programmes
* Completing assessments, care plans, and documenting progress
* Working closely with healthcare, probation, housing, and other partner agencies to ensure continuity of care pre‑ and post‑release
* Supporting the delivery of psychosocial interventions and accredited treatment programmes
You’ll also contribute to Forward’s service targets, working collaboratively with local commissioners and partner organisations to deliver measurable outcomes.
What We’re Looking For
* Experience supporting individuals with substance misuse or mental health needs
* Strong communication and organisational skills
* Ability to work flexibly, occasional evening or weekend shifts may be required
* Willingness to travel between projects within your cluster when needed
* Confidence in completing oral swab tests and acting as a second signatory for clinical work
