Role Overview
Bring your expertise in mental health and social care to a challenging and supportive environment. This role involves managing a varied caseload, providing social care for adults with a diagnosis of dementia and for older adults over the age of seventy with a severe and enduring mental health illness.
Key Responsibilities
* Investigate safeguarding concerns and conduct statutory assessments.
* Commission support services, develop support plans, and present reports to panels or in court.
* Collaborate with partner agencies, carers, and families to build networks that enable individuals to maintain independence.
* Adapt and innovate to meet the needs of clients.
Qualifications & Experience
* Good knowledge of health and social care legislation and safeguarding procedures.
* Experience working with older adults and with dementia or severe mental health conditions.
* Highly motivated, reflective, and an excellent communicator.
* Ability to meet travel requirements and work flexibly across office, client visits, and home.
Employment Conditions
