The team supports permanence planning, care reviews, and transitions for young people. Key Responsibilities: Carry out assessments, care planning, and reviews for children looked after. Develop and implement permanence plans (fostering, adoption, reunification) in partnership with carers and external agencies. Prepare and present reports for looked-after reviews, court, and legal processes. Liaise with foster carers, residential providers, education, health and other agencies.
