Overview
The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers and performing field-based duties. This role requires a valid driver’s license and access to your own vehicle due to travel involved.
What you’ll do
The Care Coordinator conducts assessments and reviews individual care plans, and supervises, instructs, and supports care staff in the field.
