We are looking for a qualified and experienced prescribing pharmacist to support patients. The post holder will be responsible for prescription and medication queries, coding medicines from outside agencies, conducting audits and reviews within the practice, including medications with shared responsibility with secondary care.

A proactive attitude, initiative, self-motivation, and drive are essential to help reduce the workload of GPs.


Main duties of the job

Working in accordance with the GPhC Code of Conduct, the post holder will work as part of a multi-disciplinary team to provide expertise in clinical medicines use and optimisation. The post-holder will deliver a high-quality clinical pharmacy service to patients in general practice, ensuring the safe, clinically effective, and cost-efficient use of drugs, using expertise in independent prescribing to support improved access to medicines.

Job responsibilities

Hours

Negotiable, 25-37.5 hours per week across 5 days, with occasional Saturday clinics.

Duties and responsibilities

* Conduct medication reviews face-to-face and via phone, assessing and treating patients using expert knowledge of medicines.
* Authorize and sign repeat prescriptions where qualified.
* Process documentation for recently discharged patients.
* Help deliver on national and practice-specific prescribing targets.
* Serve as a contact for medicine-related queries with the lead GP.
* Manage safety alerts related to drugs.
* Collaborate with care coordinators and pharmacy technicians to improve prescription management.
* Streamline medicines management systems.
* Supervise and mentor pharmacy technicians.
* Enhance prescribing quality through audits and education.
* Attend and contribute to prescribing and educational meetings.
* Ensure adherence to prescribing guidelines and update internal teams.
* Assist with CQC searches and shared care monitoring.
* Participate in clinical supervision, training, and service redesign.
* Maintain accurate records and adhere to GDPR and confidentiality policies.
* Perform other duties as assigned.


Person Specification


Qualifications

* CPPE qualification in Primary Care Pharmacy
* Independent prescribing qualification


Experience

* Experience or awareness of common acute and long-term conditions in general practice
* Ability to prioritize and problem-solve, knowing when to refer to GPs
* Ability to follow policies, procedures, and codes of conduct
* Involve patients in decisions and support adherence per guidelines


Skills & Personal Qualities

* Analytical skills for routine situations
* Excellent interpersonal, influencing, negotiation, and organizational skills
* Computer skills required; experience with SystmOne desirable
* Personal accountability, resilience, and ability to work under pressure
* Ability to work across two practices within the PCN if needed


Disclosure and Barring Service Check

This role requires a DBS check due to the Rehabilitation of Offenders Act 1975.

Salary: £40,000 to £48,000 per annum, pro-rata
