Overview
Advanced Clinical Practitioner - Trafford Crisis Response Team. We have an exciting opportunity for a qualified Advanced Clinical Practitioner (ACP) to join our supportive, dynamic Crisis Response Team in Trafford. The Crisis team is part of the Trafford Community Response Service (TCR) which also includes our Discharge to Assess and IV teams, launched September 2023. The post is based in the Trafford locality of Manchester, with potential to work across Manchester Community Response services within the City.
Joining our new integrated multidisciplinary and multi-agency team, you will provide specialist rapid assessment and interventions to the local population.
Responsibilities
* Operate as an autonomous practitioner in the Community, taking an active role in respond, stabilise and make safe elements of acute care pathways through advanced assessment, management and treatment of patients over 18 referred to TCR services.
* Deliver comprehensive healthcare in a manner sensitive to population needs, including advanced clinical assessments, history taking, diagnosis, planning and delivery of treatment; prescribe where appropriate and evaluate outcomes.
* Coordinate the delivery of urgent health care needs for acutely unwell individuals at risk of hospitalisation or requiring assisted discharge, supporting a more intensive level of care in the intermediate care environment.
* Manage direct referrals from NWAS (ambulance service), acute and primary care and provide community support across primary, secondary and social care interfaces in Trafford.
Qualifications
* Qualified Advanced Clinical Practitioner with an MSc in Advanced Clinical Practice.
* From a Nursing or AHP (Physiotherapy or Occupational Therapy) professional background.
