One Ellesmere Port Primary Care Network (PCN) delivers a wide range of innovative, community-focused services designed to improve health and wellbeing across the our town. PCN services include medicines optimisation, mental health and wellbeing support, first contact physiotherapy, social prescribing and counselling, vaccination clinics, and proactive care coordination for vulnerable patients.
As our network continues to expand and develop newservices, we are now seeking a team of Experienced General PracticeNurses to join our multidisciplinary workforce. This is an exciting opportunityto be part of a supportive environment where professional growth and patientcare are equally valued.
Main duties of the job
We are building a skilled and motivated team of Practice Nurses to deliver high-quality nursing care across our member practices. Working collaboratively with GPs, Clinical Pharmacists, Pharmacy Technicians, Mental Health Occupational Therapists, a Dementia Practitioner, Care Coordinators, Social Prescribing Link Workers, Health & Wellbeing Coaches, and other PCN staff, you will play a vital role in improving population health and supporting long-term condition management.
Dependent on the number of suitable candidatesavailable, our Nurses will work primarily in 1 or 2 GP Practices within thePCN. This will ensure that each Nurse feels embedded within a base GP Practice,whilst also being able to work collaboratively with the Nurses working in theother GP Practices, who will also be supporting proactive and preventative careinitiatives that make a real difference in the local community.

Job responsibilities
* Deliver high-quality, evidence-based nursing care
* Manage patients with long-term conditions, with a focus on patients with Asthma and COPD
* Lead long-term condition management across the PCN, supporting quality improvement and assurance initiatives
* Support health promotion and public health screening programmes, including; immunisations, vaccinations and cervical screening
* Embed population health management approaches within the PCN, targeting health inequalities and improving access
* Contribute to the delivery of national and local enhanced services
* Support, mentor, and supervise new-to-practice nurses and other healthcare professionals within the PCN
* Work collaboratively across the PCN to enhanced integration with community, secondary, and social care services
Person Specification
Experience
* Experience of working in a primary care or a PCN environment
* Experience of long-term condition management
* Experience of working autonomously
* Experience of infection prevention and control measures
* Experience of quality improvement / initiatives, i.e., benchmarking
* Experience of working as a practice nurse or community nurse
* Experience of managing elderly and housebound patients

Qualifications
* Registered Nurse (Nursing and Midwifery Council)
* Level 6 diploma or postgraduate certification in long-term conditions care and / or public health initiatives
* Nurse prescriber
* Family planning qualification
* Mentor or teaching qualification

* Ability to record accurate clinical notes
* Ability to work within own scope of practice and understanding when to refer to GPs
