We have a vacancy in our services in South Tyneside, where we support adults with various disabilities and complex needs.

Key Responsibilities
* Staff will contribute to the development of the Service provided through active participation in Supervision Sessions, Appraisals, Team Meetings and other appropriate forums.
* Staff will continue to support people to improve their health, to improve the care provided and experience received by the people supported.
* To support people with their finances as appropriate ensuring that all records are kept in line with relevant policies and procedures.
* To support people in all aspects of their daily lives, actively promoting independence by assessment and planning in conjunction with his or her Person-Centred Plan
* To ensure that the systems of assessment and planning are maintained in line with policies and procedures. To maintain all records and attend relevant meetings as required.
* To participate in a keyworker system where relevant
* To work to a professional standard in a manner that promotes positive relations
* To practise non aversive strategies to adverse behaviour at all times and to support people who express their frustrations and needs through behaviours of concern by using appropriate strategies, management guidelines and intervention frameworks as specified by the Person Centred Plan.
* To support and follow behaviour support plans, using positive behaviour support strategies and de-escalation techniques to manage challenging situations effectively and to improve quality of life.
* To support the person in all aspects of running their home.
* To support the person in maintaining their dietary needs and promoting a healthy lifestyle.
* To support the person on any short breaks or holidays they may wish to take.
* To report any achievements, incidents, accidents, illnesses, occurrences in accordance with Policies and Procedures
* To adhere to all Health and Safety and Risk Assessment Policies, Procedures, Guidance and Protocols.
* To undertake any specific duties which may be delegated, which are within Staff members level of competence and experience
* To actively network in the Community to ensure the people supported have strong links and experiences, valued Social Inclusion and Integration.
* To support the person in all of their Physical, Intellectual, Social and Emotional needs and requirements.
* Actively support community inclusion, social value, and wellbeing in line with Service Specification and Council requirements as per the High Level Needs Framework for South Tyneside.
* Monitor complex health conditions and act on early deterioration, and escalate promptly to clinicians and urgent care when required.
* Reduce barriers to inclusion: advocate where possible and consider reasonable adjustments so people can use community services and lead safe and healthy lives.
Experience and skills:
* Excellent communication skills. Experience of building positive working relationships with people who use the service and their families, staff, and other health and social care professionals
* Ability to support the people we support with all aspects of their daily living in a manner that respects their dignity, is non-judgmental, and promotes their independence, choices, and privacy
* Experience of ISL services, risk assessment, and person-centred care and support
* Good administrative skills
* Ability to maintain clear written and electronic records
* Experience in financial management is desirable
* Ability to follow policies, procedures, and instructions
* Must have Level 3 in Health and Social Care or equivalent, or be willing to work towards
* Must have a Care Certificate qualification or equivalent
* Knowledge of STOMP would be desirable
