Community Flow, Delayed Discharge and Integrated Discharge Hub Manager

Are you an experienced leader ready to drive innovation and improvement in patient flow and discharge across Fife? We are seeking a manager to lead our Community Flow, Delayed Discharge, and Integrated Discharge Hub services.

About the Role:

You will have strategic and operational responsibility for managing and developing complex, multi-agency services that support timely hospital discharge and prevent unnecessary delays. You’ll lead a multidisciplinary team, oversee a significant budget, and play a key role in service redesign and winter planning.

Key Responsibilities:

• Lead and develop the Community Flow and Integrated Discharge Hub teams

• Ensure high-quality, person-centred care and improved outcomes for people leaving hospital

• Drive service redesign, including the Home First model

• Manage performance, resources, and budgets (approx. £2m)

• Build strong partnerships across health, social care, and the third sector

About You:

• Educated to degree level (or equivalent), ideally with a management qualification

• Significant management experience (minimum 5 years) in health, social care, or related sectors

• Proven leadership, change management, and communication skills

• Experience in service redesign and managing multidisciplinary teams

• Strong knowledge of current health and social care policy

However, this is only possible if the employer is a licenced Sponsor, and if the post does not fall below the current minimum salary threshold or ‘going rate’. It is ESSENTIAL that you have checked that you either already have an appropriate right to work in the UK or that the post would be eligible to be sponsored BEFORE submitting your application form.

Due to legislative changes from 1 April 2025, this post may require a different level of criminal records check done than is currently the case. If the post is assessed as a "regulated role", your appointment will be subject to joining the Protecting Vulnerable Groups (PVG) Scheme.
