Job Overview An exciting opportunity has arisen for a GP to join the Living Well Service Multi-disciplinary team in Betsi Cadwaladr University Health Board (BCUHB) on a permanent basis. We are looking for someone who enjoys working as part of a skilled multidisciplinary team, supporting and collaborating. The team includes a Consultant Therapist and Psychologist, Clinical Psychologists, Assistant Psychologists, Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists and support staff. The service supports people presenting with long COVID, other post viral conditions, ME/CFS, functional neurological disorders, breathing pattern disorder and those who present frequently to services. We are an award winning, friendly and supportive team who are research active, with a passion for co-production with our partners with lived experience.
We are a pan North Wales service; this role will be required to be undertaken each Wednesday at 2 sessions per week on top of the current GP role.
Primary Responsibilities The post holder will undertake clinical work within the BCUHB Living Well Service, working directly and indirectly with patients, families in outpatient clinics and in their own homes when necessary. As a key member of the Multi-Disciplinary team, the GP will provide clinical support to the MDT in the triage, assessment and holistic care planning for the client group. They will contribute to service evaluation and planning. They will provide support for Primary Care colleagues through education and clinical leadership, and participate in ongoing service design and evaluation. They will be expected to support the non-medical prescribers in the team.
Regular review of the literature will be integral to the role, to inform the advice given to patients, maintaining credibility with well-informed patient groups.

Qualifications and Experience Essential Criteria
Fully qualified GP with GMC registration
Evidence of annual appraisal and revalidation (when appropriate)
General practice (Vocational Training Scheme) trained
On/eligible to be on a medical performers list
Enhanced CRB check
UK driving licence
Current CPR certificate
UK work permit (if required)

Evidence of continued professional development

Successfully completed General Practice Specialist Training
Appropriate range of previous experience in hospital posts
Clinical and Information Governance

Computer literacy
Multi-morbid – Long Term Condition Management
Desirable Criteria
Experience of working a variety of clinical software systems
