About The Role
As a Social Worker within the Children and Young People\'s Disability Service (CYPDS) you will manage a caseload of children on child-in-need plans, child protection plans, and some looked after children subject to proceedings. You will work with colleagues in Health and Education Services, lead on creating short breaks support packages, and develop skills in preparing young people for adulthood, including transition to adult services and parent carer needs assessments.
You will be involved in all tasks associated with children under statutory frameworks and will monitor care packages for effectiveness.
Responsibilities
* Manage a caseload of children on child-in-need and child protection plans, and some looked after children subject to proceedings.
* Undertake tasks associated with children under statutory frameworks.
* Work closely with Health and Education Services to coordinate care.
* Lead on creating short breaks support packages for families.
* Develop skills in supporting young people in preparation for adulthood and transition to adult services.
* Conduct parent carer needs assessments as required.
Qualifications and Skills
* Qualified Social Worker with a Social Work degree or recognised qualification and registered with Social Work England.
* Have completed the Assessed and Supported Year in Employment (ASYE).
* Evidence of safeguarding vulnerable individuals and complying with statutory functions, policies and procedures with timely documentation.
* Compassionate and empathetic approach; effective with individuals and families affected by disabilities.
* Strong assessment, analytical and risk management skills; excellent communication; ability to plan and monitor care packages.
* Ability to demonstrate understanding, patience and sensitivity when interacting with service users.
