Job Description
To participate in the provision of high-quality care. This will involve assessing, supporting, and monitoring service users experiencing mental health issues.
It will involve working in a variety of community settings; liaising and co-working with other statutory and non-statutory services.
It will involve working with carers and families of service users, including visiting in their homes, clinics, or environments such as hostels, supported living, and residential settings.
* To work as a Care-coordinator or Lead Professional within the team in planning, delivering, coordinating, and evaluating assessments and formulations.
* To work closely and collaboratively with other team members, including therapists, medical professionals, nurses, allied health professionals, and support staff.
* To carry out complex, person-centered assessments.
* To ensure all care provided is evidence-based, considering the physical, cultural, psychological, and spiritual needs of service users.
* To assess care needs, devise care programs and packages, including risk assessments and interventions like Dialog+, and to implement and evaluate these in collaboration with service users and their carers.
* To support and mentor less experienced staff, support workers, and students on placement within the team.
* To undertake duties to extend clinical and managerial expertise as delegated by the Team Manager.
* To undertake comprehensive outcome monitoring of all referrals.
