An exciting opportunity has arisen within the newly formed Sea City Partnership for keen and committed individuals to joinour friendly team to develop medicines optimisation services within ourpractices.

The post holder will play a key role in improving safe and cost-effective prescribing, enhancing patient outcomes,reducing health inequalities, and supporting public health initiatives.

We are looking to recruit a PharmacyTechnician (Salary AFC Band 5 equivalent pro-rata with NHS Pension available)to enhance the care of our patients.

The posts will complement the existingPharmacist Team, who will provide professional support and development.

We will welcome applications from all sectors of Pharmacy, and you will be fully supported with a comprehensive induction andmandatory enrolment in the Primary Care Pharmacy Education pathway provided byCPPE if you have not already completed this.

Main duties of the job

3.1.1Providing specialist pharmaceutical services, professional advice anddirect support to Drs, Nurses, HCAs, ANPs, Reception/Administrative Staff andthe wider multi-disciplinary ARRS team to promote the safe, effective andhigh-quality, cost-effective use of medicines.

3.1.2Liaising with patients, clinical and non-clinical staff, practice,hospital and community pharmacists/ technicians and patient's carers to improvethe pharmaceutical care of patients.

3.1.3Undertaking medicines reconciliation inaccordance with practice protocols and within clinical competence to updatepatients medical records accordingly referring on any discrepancies promptlyto the clinical pharmacists or GPs.

3.1.4Assisting clinical pharmacists in Structured Medication Reviews (SMRs),including arranging required monitoring and carrying out preparatory work.

3.1.5Supporting the clinical pharmacists with long term condition reviewsthrough lifestyle and medication advice.

3.1.6Responding to medication queries from patients, carers, and staff,providing counselling where appropriate.

The above is not exhaustive. The postholder may be required to undertake other activities commensurate with theexpected levels of responsibility and scope of the role, within levels ofattained competence. This will be dependent upon factors such as workload and staffing levels, annual leave cover.

Job responsibilities

3.1 The post holder will be responsible for:

3.1.1Providing specialist pharmaceutical services, professional advice anddirect support to Drs, Nurses, HCAs, ANPs, Reception/Administrative Staff andthe wider multi-disciplinary ARRS team to promote the safe, effective andhigh-quality, cost-effective use of medicines.

3.1.2Liaising with patients, clinical and non-clinical staff, practice,hospital and community pharmacists/ technicians and patient's carers to improvethe pharmaceutical care of patients.

3.1.3Undertaking medicines reconciliation inaccordance with practice protocols and within clinical competence to updatepatients medical records accordingly referring on any discrepancies promptlyto the clinical pharmacists or GPs.

3.1.4Assisting clinical pharmacists in Structured Medication Reviews (SMRs),including arranging required monitoring and carrying out preparatory work.

3.1.6Responding to medication queries from patients, carers, and staff,providing counselling where appropriate.

3.1.7Liaising with community pharmacy teams for items which are out ofstock, supporting compliance with medication and promoting lifestyle advice(smoking/alcohol).

3.1.8Promoting safe and effective prescribing, reduce medicines waste, andsupport repeat prescribing systems.

3.1.9To work with reception staff and medicines managers to help managerepeat prescription requests, resolving queries where possible and producingprescriptions for clinical review.

3.1.10Supporting the team with QOF, MOIS, NICE guidance implementation,antimicrobial stewardship, and prescribing audits.

3.1.11Training and supporting admin staff and pharmacy colleagues in safeprescribing processes.

3.1.12Participate in audits, service evaluation, and policy development to improve medicine use.

3.1.13Monitoring and developing recall systems to ensure that all patientshave appropriate therapeutic drug monitoring.

3.1.14Responding to medicines related MHRA safety alerts.

3.1.15Assisting in the appropriate monitoring of high-risk medicationsthrough patient safety searches.

3.1.16Processing and reviewing shared care agreements received from other healthcareproviders.

3.2 Communication Responsibilities:

3.2.1Work collaboratively with multidisciplinary teams, contributing to patient group meetings and service discussions.

3.2.2Provide information and support to patients, carers, and professionals, tailored to their needs and understanding.

3.3 Knowledge, Education, and Training Responsibilities:

3.2.1Participate in delivering training to staff and patient groups.

3.2.2Take responsibility for CPD and revalidation requirements with theGPhC.

3.2.3Identify and undertake training to support service needs, including ITsystems (System One, AccuRx, Anima).

3.3 Management and Leadership:

3.3.1Manage own workload effectively, working independently and as part of ateam.

3.3.2Support pharmacists with QOF and MOIS and other medicine-relatedtargets.

3.3.3Contribute to audits, data collection, and reporting to commissioners.

3.3.4Supervise and train administrative and trainee staff as required

3.4 Quality and Service Improvement:

34.1Identify risks and ensure safe practice in medicines management.

34.2Contribute to evidence-based practice, audit, and research.

34.3Participate in service improvement initiatives and development ofclinical standards and protocols.

34.4Promote patient and public involvement to inform service improvements.

Person Specification


Experience

* Experience of working in GP practice or NHS provider as a pharmacy technician, hospital or community pharmacy.
* A sound knowledge of the working of General Practice and NHS Primary Care.
* Experience with clinical computer systems such as SystemOne.
* Experience of patient-facing medication counselling role.
* Experience in coordinating and prioritising daily workload.
* Problem solving/decision making

Qualifications

* NVQ level 3 in Pharmaceutical Services
* Pharmacy Technician Registered with General Pharmaceutical council.
* Evidence of CPD
* PCPEP
* Full driving license and access to a car
