What You’ll Be Doing
As a Care Coordinator, you’ll be at the centre of everything — supporting your team, solving problems, and ensuring care runs smoothly every day.
You’ll:
Create and manage staff rotas, ensuring safe and efficient care delivery
Match carers to clients based on needs, preferences, and location
Handle new referrals and support care package set-ups
Communicate with clients, families, and healthcare professionals
Support care planning, reviews, and risk assessments
Carry out care worker supervisions and spot checks
Keep records accurate and up to date
Support team meetings, KPIs, and day-to-day coordination
Take part in the on-call rota (including alternate weekends)
No two days are the same — but you’ll always have the support of an experienced and approachable management team.
What We’re Looking For
We’re open to different backgrounds — what matters most is your attitude, organisation, and passion for care.
You might be:
A current Care Coordinator
A Senior Carer or Team Leader looking to progress
Someone with strong scheduling, admin, or care experience
You’ll need:
Experience in care (domiciliary preferred)
Strong organisation and time management skills
Confidence using IT systems
A full UK driving licence and access to a vehicle
NVQ Level 2 in Health & Social Care (or working towards)
