We are currently recruiting for an Twilight Review & Assessment Officer who will complement our team. Your base of work may change if the needs of the business change.
Every working day will be different. You will be working evenings and weekends.
Key Responsibilities include:
* Support the team to deliver personal care with an enabling approach,
* Work with people in service,
* Work alongside other Provider Services and Social Work teams,
* Undertake Care Act assessments and devise Care Plans/Personal profiles that are strengths based, and person centred and tailored to meet individual needs.
* Liaise with individuals, their families, other professionals, and agencies involved in the provision of care and support, to ensure positive outcomes, independence, and well-being
* Support, guide and supervise a team of enthusiastic and caring Support Workers, to deliver a high standard of quality care, including carrying out observations.
* Complete Care Act Reviews
