About the Discharge to Assess Service Our team plays a vital role in: Supporting patient flow across the hospital system Identifying community-based solutions Signposting and advising citizens and carers Conducting Care Act and Mental Capacity assessments The team help improve health outcomes and experiences for citizens in Birmingham What We’re Looking For We’re seeking Qualified Social Workers who: Hold a CQSW, DipSW, CSS or equivalent Social Work England-recognised qualification Registered with Social Work England Have experience working with vulnerable adults (hospital experience is desirable but not essential) Can work effectively in a fast-paced, multidisciplinary environment Is confident in making decisions under pressure and managing complex needs Is committed to equality, continual professional development, and service improvement Why Join Us?
