Job summary
Forest Heath Primary Care Network (PCN) has an exciting opportunity for a hardworking, enthusiastic and innovative pharmacist to join our primary care network. You will be based at one of our 8 GP practices on a rota, and will be required to travel between all of the GP practices within our PCN. This role is offered on a full or part-time basis, for up to hours per week.
Main duties of the job
1. Clinically assessing and treating patients, using expert knowledge of medicines, for specific disease areas;
2. Providing specialist expertise in the use of medicines and medicine information ( side-effects, dosage);
3. Providing pharmaceutical medication review and health education advice to patients, by face-to-face, telephone or notes-based review, as agreed with the practice;
4. Providing leadership on person-centred medicines optimisation and quality improvement;
5. Contributing to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes;
6. Processing specialist treatment letters;
7. Supporting practice dispensary;
8. Processing hospital medication discharge letters;
9. Acting as a key contact, for all medication related queries;
10. Conducting spirometry, child immunisation, travel vaccination, diabetic foot checks and NHS health checks;
11. Conducting care-planning, including mental health, admission avoidance schemes, and end-of-life patients.
12. Undertaking minor ailments triage and directing patients, based on clinical need;
13. Capturing relevant patient health information at all consultations including contributing to the quality outcomes framework;
14. Monitoring patients with complex, long-term conditions such as hypertension, diabetes and COPD;
15. Taking a central role in the clinical aspects of shared-care protocols, clinical research with medicines and liaison with specialist pharmacists.

Essential
16. Completion of a degree in pharmacy and registration with the General Pharmaceutical Council;
17. Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this;
18. Qualified independent prescriber, or working towards this;
19. Member of the Royal Pharmaceutical Society (RPS).

20. Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice;
21. Ability to demonstrate general pharmaceutical skills and knowledge in core areas;
22. Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions;
23. Ability to demonstrate accountability for delivering professional expertise and direct service provision as an individuals;
24. Ability to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence, when appropriate;
25. Ability to follow legal, ethical, professional and organisational policies, procedures and codes of conduct;
26. Ability to assist patients in decisions about prescribed medicines and supporting adherence as per NICE guidance;
27. Non-judgemental attitude and ability to be flexible and adaptable;
28. Self-motivated with excellent communication skills;
29. Understand the need for patient confidentiality.
