Home Health Occupational Therapist (OT) - PRN

Key Responsibilities
* Assess the patient’s functional status (muscle function, endurance, visual coordination, communication skills, self-care, work capacity, etc.).
* Evaluate the home environment for hazards or barriers to independent living and identify necessary equipment.
* Collaborate in developing a comprehensive care plan.
* Design a treatment program with goals for improved function and communicate the plan to the referring physician and other team members.
* Teach or retrain patients in daily activities lost due to illness or injury, following agency policies.
* Fabricate splints and instruct patients on using adaptive equipment to enhance function.
* Provide training in the use of prosthetic and/or orthotic devices.
* Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
* Participate in in‑services, case conferences, and meetings to promote coordinated care with healthcare team members.
* Work alongside physicians and discharge planners to provide health education to patients and families/caregivers.
* Identify patient and family/caregiver needs for additional services and make appropriate referrals.
* Confirm daily visit schedules to ensure seamless coordination with other staff.
* Supervise Certified Occupational Therapy Assistants in accordance with agency policies and state regulations.
Qualifications
* Must have a current OT license (state‑specific).
* Graduate of an accredited occupational therapy program recognized by the American Medical Association and the American Occupational Therapy Association.
* Registered with the National Registration Examination of the American Occupational Therapy Association.
* Valid BLS certification.
* Minimum two years of OT experience (home health experience preferred).
* Excellent communication, organizational, and time management skills.
* Must have a valid driver’s license and access to a reliable and insured vehicle.
* Ability to visit homes that may not be wheelchair accessible.
* Confirm daily visit schedules to ensure seamless coordination with other staff.
* Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
* Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
