We are seeking an experienced Children's Mental Health Practitioner to join our Canvey Island PCN for a 12-month fixed-term position.
As part of the NHS long-term plan, local areas are realigning community mental health services with primary care networks, creating integrated models of primary and community mental health care.
PCN's Aim To
1. Have a key coordinating role at the heart of GP practices and be a fully integrated member of the multidisciplinary team regarding response and management pathways.
2. Provide combined consultation, advice, triage, and liaison functions according to agreed protocols.
3. Support children and young people with mild to moderate mental health issues through evidence-based interventions.
PCNs (Primary Care Networks) are designed to support children and young people. The Mental Health Senior Practitioner will be responsible for assessment and caseload management within the GP PCN, including triaging cases, conducting full assessments, and managing their workstream. They will handle a caseload of CYP with mild to moderate MH needs and work closely with the operations manager and Clinical & Strategic Lead to ensure effective delivery of the PCN initiative in Essex.
This CYP MH Practitioner role involves supporting the development and delivery of the PCN service, implementing evidence-based low-intensity interventions in GP services, and liaising between professionals. The post involves working with GPs to support children and young people experiencing mild to moderate mental health difficulties and their families.
The ideal candidate will have experience working with children and young people within this framework, delivering individual and group interventions, managing a clinical caseload, and meeting service targets.

Starting with NELFT
New starters will join on the first Monday of each month and undergo a comprehensive two-week induction, including mandatory training, systems training, and equipment allocation. They will also have opportunities to meet the executive team and participate in engagement and wellbeing sessions at our head office in Rainham, Essex.
Probationary Period
This role is subject to a probationary period. Internal applicants may be exempt based on their current status.

High-Cost Area Supplement
This position attracts a 5% supplement of the basic salary, with a minimum of £1,192 and a maximum of £2,011 per annum (pro-rata).
