Contract Duration: 3 Months Full Time
Embark on an exciting journey with us as a Social Work Advanced Practitioner in our Early Intervention Team based in Stevenage. You will lead the charge in peer development and practice, supporting and supervising a team to elevate their skills and knowledge in line with vital legislation such as The Care Act and Hertfordshire''s Connected Lives principles.
Responsibilities
* Lead and supervise a team of Community Care Officers, ensuring high-quality delivery of statutory social care services.
* Provide urgent social care assessments and interventions to support individuals remaining in their usual place of residence rather than being admitted to hospital.
* Collaborate with health professionals from the Early Intervention Vehicle to assess and support adults at risk, focusing on safety and wellbeing.
* Build links with local communities and undertake multi-agency work alongside partner organisations, professionals, individuals, families, and carers.
* Support individuals in accessing community and voluntary support, while also giving advice and guidance to unpaid carers.
