Overview
An exciting opportunity for a Physiotherapist with experience of, or interest in, community based working across the Bury Town, Haverhill, Mildenhall and Newmarket Integrated Neighbourhood Teams. The teams operate in a variety of settings across West Suffolk, covering historic market towns and surrounding villages. You will work with a range of health professionals and agencies including Adult Social Care, in an integrated way to provide the best outcomes for people within our localities.
The successful applicants will join a dynamic, friendly team of therapists, community nurses and social care staff. You will provide Physiotherapy to patients in the community with the autonomy to manage your own caseload, supported by the wider multidisciplinary team within the hospital and across the West Suffolk Alliance.
Responsibilities
* Provide physiotherapy to patients in the community, with autonomous management of a caseload and collaboration with a wider multidisciplinary team.
* Deliver therapy for patients with long-term conditions and a responsive, same-day service to facilitate hospital discharges.
* Carry out joint health and social care assessments of need in the community.
* Utilise a wide range of physiotherapy skills with high level assessment abilities.
* Develop your role in line with clinical interests; access training and education to support this.
* Supervise and mentor junior staff and students; participate in ongoing learning and development within the team.
* Role available in both full-time and part-time capacities.
* Frequent travel is essential; hold a full, current driving licence with access to personal transport or meet mobility requirements by other suitable means.
* Work within a 7-day service during core hours 8am to 6pm.
Qualifications and Experience
* High-level assessment skills with the ability to work independently within a multidisciplinary team.
* Experience in neurological, musculoskeletal, frailty, falls, dementia and orthopaedics is advantageous.
* Demonstrated interest in developing within community-based physiotherapy and continuing professional development.
