Overview
This post is available as an internal 12 month secondment due to the substantive postholder being seconded to another team to cover maternity leave. The postholder will undertake a key worker role, alongside an assessment and formulation role within the team.
The Living Well Service enables timely access to mental health services when people need it most and provides support for people who might not traditionally meet the threshold or criteria of existing mental health services. The team is multidisciplinary and is operated in partnership with Family Action (BAND), our third sector partner.
The postholder will primarily cover South Bolton and will be based at Farnworth Town Hall.
Responsibilities
* Undertake a key worker role, alongside an assessment and formulation role within the Living Well Service.
* Cover South Bolton as the primary area of responsibility and be based at Farnworth Town Hall.
* Triaging and signposting referrals, liaising with a multitude of agencies and professionals.
* Engage with community services and third sector organisations in Bolton; experience and knowledge in these areas are beneficial.
