A fantastic, new opportunity has arisen to become an Complex Case Nurse (Alcohol), improving choice and control for individuals who have complex needs may be house bound, homeless, have co-existing physical health concerns or who find engaging with services difficult.
Our teams are multi-disciplinary, they consist of a service manager, consultant psychiatrists, doctors, nurses, recovery staff, volunteers, administrators, and peer mentors, all focused on bringing the individuals using our services the best possible support & interventions.

Full Time Salary: £37,184.90 - £43,384.61*

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
Responsibilities
Main duties of the job
Our Complex Case Nurses play an integral part in Change Grow Lives mission to help people change the direction of their lives, grow as individuals, and live life to its full potential. Working as part of a multi-disciplinary team, they offer specialist clinical interventions to people who use our services.
The post holder will be supporting our hard-to-reach clients with complex needs, helping them to access the support and treatment they need alongside addressing their substance misuse issues. Therefore must hold a full UK driving license and have access to a car during working hours that is covered by business insurance.
The Complex Case Nurse role is a varied one, the successful candidate will be an integral member of a multidisciplinary team, offering specialist clinical interventions to people who find it difficult to engage with services.
Your role includes:
1. Using your nursing skills/knowledge to carry out physical and psycho-social assessments for the complex needs’ clients in East Sussex.
2. Providing clinical /therapeutic treatments based on best possible evidence that will improve health outcomes.
3. Provide screening for blood borne viruses (BBV), through dry blood spot testing.
4. Working within and consolidating integrated care and treatment pathways for individuals infected with BBV/s (Hepatitis B, Hepatitis C and/or HIV). Provide Hepatitis A and or B vaccinations when indicated.
5. Working in partnership with other community teams/voluntary organisations to ensure the seamless delivery of care.
6. To contribute to CGL Naloxone Strategy by training staff and individuals in the use of Naloxone.
7. Offer wound care assessment and basic wound dressing to individuals who use our services. Organise signposting to other services as required and ensure clear pathways into other treatment arenas as appropriate.
About You:
8. First level registered nurse with recent relevant post qualification experience and a current PIN.
9. Sound and demonstrable knowledge of harm reduction and health promotion interventions.
10. Ability to assess and formulate a treatment plan for service users and formulate written reports and letters to professionals, as necessary.
11. An understanding of and commitment to safeguarding best practice.
12. An understanding of the importance of professional integrity in relationships with service users, peers, and other relevant professionals.
13. An understanding of information governance processes and a commitment to follow and apply all necessary safeguards
