Job overview

The post holder will, with the support of the Pharmacy Senior Management Team, lead all aspects of the medication safety agenda across the Trust and act as the Trust Medicines Safety Officer. They will work as part of the Pharmacy Board Team to deliver the medicines’ clinical governance plans which support safe and effective Pharmacy and medicines optimisation services to patients under the care of Calderdale and Huddersfield NHS Foundation Trust and other Trusts under service level agreements. This involves close liaison with multidisciplinary teams working across the Trust. They will develop education and training materials including reports relating to the management of medicines in the Trust and undertake research and audit to support innovative practice and sustain robust medicines management systems and processes.


Main duties of the job

* Lead the Medication Safety agenda and be responsible for the monitoring systems and provision of information to support a reduction in avoidable harm resulting from inappropriate medicines use across the Trust including a medicines safety dashboard.
* Act as a role model and expert in medication safety ensuring a consistently high level of pharmacy input in line with corporate objectives and NHS England policy.
* Manage the Pharmacy Medication Safety Team to ensure that the medicines clinical governance agenda is delivered in line with Trust priorities.
* Undertake research and audit to mitigate the risk associated with medicines use.
* As part of the Pharmacy Management team contribute to the strategic development of pharmacy services and lead by example to enhance the Pharmacy Service reputation both at a local and national level.
* Ensure medication safety is a common theme across all elements of business planning for the Trust.
* Contribute to the local, regional and national agenda with respect to medication safety liaising with national, regional and local groups to share, learn and minimise medication errors.
* Be responsible for the production, implementation and maintenance of a Trust-wide Medicines Code.
* Provide support to the Trust Accountable Officer for Controlled Drugs.
* Support safe handling, prescribing and administration of Medical Gases.


Person specification


QUALIFICATIONS / TRAINING

* MPharm or equivalent Degree in Pharmacy approved by General Pharmaceutical Council
* Registered Pharmacist with the General Pharmaceutical Council
* Specialist training and competencies in medication safety or governance or relevant short courses taught at postgraduate level


KNOWLEDGE, EXPERIENCE & EXPERTISE

* 1 year pre-registration training
* Highly specialised knowledge of medicines related governance with attention to detail
* Extensive experience as a practicing Senior Clinical Pharmacist within a Hospital Pharmacy with demonstrable experience in managing medication safety
* Evidence of high-level investigational skills with the identification of key learning
* Demonstrate the ability to lead and influence others and implement change
* Working knowledge of strategies to implement national guidance for safe medicines use and prevention of medication related Never Events
* Pharmacy Practice Research and Clinical Audit
* Ability to prioritise workload and meet deadlines whilst paying attention to detail
* Ability to work under pressure
* Ability to work on own initiative and as part of a team
* Demonstrate commitment to CPD
* Able to fulfill the health requirements of the post as identified in the Job Description, taking into account any reasonable adjustments recommended by Occupational Health
* Must be eligible to work in the UK
* Ability to work in various locations throughout the network of services provided by Calderdale and Huddersfield NHS Foundation Trust
* Management of staff and /or drug budgets
* Ability to multitask and influence others
* Evidence of leading or participating in training and education of others

* Able to demonstrate significant relevant experience of clinical pharmacy practice, and management of staff
* Excellent interpersonal and communication skills
* Self-motivating and ability to influence, challenge and negotiate with medical, nursing and pharmacy staff
* Planning and organisational skills
* Able to effect and manage change
* Problem solving and analytical skills
