What You Will Do:
Assess and evaluate service users’ needs to determine appropriate aids and adaptations.
Develop, review, and prescribe major adaptation plans to enhance the independence and quality of life for service users.
Collaborate with a multidisciplinary team to implement adaptation strategies, ensuring a cohesive approach.
Provide guidance and support to service users and their families about available adaptation options.
Maintain detailed records and reports to ensure a high standard of service delivery.
An essential requirement for this role is HCPC registration, ensuring you meet professional and ethical standards in your practice.
