Overview
Band 6 Specialist Occupational Therapist. Barnstaple Community Health and Social Care Team welcomes experienced Band 5 OT seeking new rotations or Band 6 OT seeking community work. This role focuses on rehabilitation in patients\' homes and is delivered by a supportive multidisciplinary team in central Barnstaple, working with social care and community nursing teams.
Experience in rehabilitation across orthopaedic, amputees, neurological, mental health and physical health conditions is welcome.
Responsibilities
* Hold and manage your own caseload in the community.
* Provide education and intervention to enable self-management and goal setting in partnership with patients.
* Aim to avoid hospital admissions and support timely discharges from hospital.
* Mentor and supervise other clinicians; reflect; contribute to CPD portfolio; lead on student education.
* Collaborate with the community multidisciplinary team and wider partners; plan and attend community visits (some lone, some joint with therapy or wider team).
* Document care using the My Care system; record live during patient contact; mobile phone and laptop provided.
* Engage with local GP practices and services (Litchdon, Queens, Brannams, Fremington Medical centres) and visit patients in Barnstaple and surrounding areas.
Working Pattern and Location
The role is part-time at 22.5 hours per week, spread across 3 or 4 days and must include a Friday. There are additional on-call and weekend working responsibilities. Based at Barnstaple Health Centre, with co-location with social care and community nursing.
Pay and Benefits
Pay scheme: Agenda for Change; Band 6. Salary: £38,682 to £46,580 per year pro rata. Contract: Permanent. Location: Barnstaple Health Centre, Barnstaple, Devon EX327BH.
