Job Description:
An exciting job role has recently become available at Lancashire. We are looking for a new member for their Hospital Discharge Team.
What will your responsibilities be?
You will evaluate the medical, social, and emotional needs of patients to develop comprehensive discharge plans. Collaborate with healthcare providers, community agencies, and insurance companies to arrange for home care services or transfers to other facilities. Provide counseling and emotional support to patients and their families during the discharge process. Educate patients and their families about available resources, support systems, and their rights. Maintain accurate records of all interactions and discharge plans in the patient’s medical record.

Qualifications and Experience:
To be eligible for this role, you must have Social Work England registration, be eligible to work in the UK, hold a full UK driving licence, and possess post-qualified experience. A degree or equivalent qualification in Social Work is also required.
