The project lead will provide a management function to ensure effective service delivery:

· They will establish and nurture good relationships with a variety of stakeholders across the health and social care system.

· They will be responsible for supporting and line managing a multi-disciplinary team.

· They will ensure appropriate planning and allocation of work and organising rotas.

· As the area service lead, they will be responsible for implementing and ensuring adherence to Kaleidoscope policies, procedures, and protocols including appropriate risk assessment and risk management tools.

· The will support the co-production of service delivery model encouraging community engagement, student placements volunteering and Peer2Peer delivery within the Rhyl region.

· The post holder should be committed to evidence-based practice, sound clinical governance and continual improvement, and have a desire to firmly embed these principles within the team.

Key Duties and Responsibilities

· Ensure specialist substance use provision is available to individuals experiencing unmet need within the Rhyl region.

· Develop assertive outreach provision based on harm reduction and co-production to reach out to those not engaged in treatment services.

· Monitor and ensure accurate record keeping on case management systems across the service.

· Adhere to health and safety, vetting, and security procedures/polices.

· Compile reports on attendance, engagement, and recovery from substance use, and submit accurate and reflective assessments for safeguarding and unmet needs care planning.

· Collaboratively develop and implement effective and flexible systems and structures.

· Contribute to colleague recruitment and onboarding.

· Ensure service user involvement and co-production is at the heart of service delivery.

· Ensure appropriate safeguarding and data protection principles are adhered in working practice.

· Respond in a timely and appropriate manor to complaints feedback and concerns

· Implement robust systems for reporting any untoward incidents, complaints, and safeguarding concerns.

· To attend and report to safeguarding reviews, meetings, and conferences e.g. Multi Agency Risk Assessment Conferences (MARACs) and Fatal / Non-Fatal Drug Poisoning Review Panel.

· Promote information regarding substance use and associated lifestyle behaviours that reduces the risk of overdose, reduces the spread of blood borne viruses, promotes healthy living, and minimises the potential risks and impact of drug related crime and disorder on the wider community. Including the delivery of Needle Syringe Programme (Needle Exchange), Blood Borne Virus testing, vaccinations, and Naloxone training.

· Liaise closely with health and social care service providers such as Betsi Cadwaladr University Health Board to ensure seamless transitions to and statutory and voluntary sector substance misuse services across North Wales.

· Assist with the production of leaflets, marketing material and web pages.

· To ensure appropriate crisis intervention is available where necessary and appropriate

· Promote and encourage the utilisation of self-help manuals.

· To undertake other reasonable duties as requested, but which may not be specified within the job description

Job Types: Full-time, Fixed term contract

Contract length: 12 months

Pay: £32,000.00 per year

Ability to commute/relocate:

* Rhyl LL18: reliably commute or plan to relocate before starting work (required)
