Responsibilities
* Surgery consultations
* Online consultations
* Telephone consultations and queries, visiting patients at home (home visiting service provided by the PCN)
* Checking and signing repeat prescriptions and handling related queries and paperwork
* Making professional autonomous decisions in relation to presenting problems
* Assessing health care needs of patients with undifferentiated problems
* Screening disease risk factors and early signs of illness
* Developing care plans in collaboration with patients in line with Practice protocols
* Providing counselling and health education
* Admitting or discharging patients from the caseload and referring to other providers as appropriate
* Recording clear and contemporaneous notes; data for audit
* Compiling and issuing computer-generated acute and repeat prescriptions in a timely and safe manner

Qualifications
* Essential: Fully qualified and registered General Practitioner
* Desirable: Special interests; Training and mentoring

* Desirable: Knowledge of Systm1; Special interest
Experience
* Essential: Experience of working in general practice
* Desirable: Experience of working within the BANES area

About Elm Hayes Surgery
Elm Hayes Surgery provides primary care services to its patients from modern premises in Paulton. We are part dispensing with an on site complementary therapy suite. We have support from an in-house team including Nurses, Clinical Pharmacists, Administrators and external Community Staff. We are fully computerised using Systm1 and have a digital triage model using Accurx. Elm Hayes Surgery is part of Three Valleys Health Primary Care Network Limited which supports the practice in various ways including ARRS staff. Home visits are operated by the Three Valleys Health Home Visiting Service and Elm Hayes Surgery participates in the Primary Care Network.
