Role Overview
We are recruiting qualified Social Workers on behalf of a local authority in the West Midlands to support Adult Social Care – Intermediate Care Services. This role is offered on a hybrid basis, with onsite working across community and hospital settings.
This is a contract opportunity providing cover for a vacant post within a busy and well-established service.
Key Responsibilities
* Managing adult social care caseloads within Intermediate Care Services
* Completing assessments, reviews, and care planning
* Supporting hospital discharge and community-based interventions
* Working collaboratively with health professionals and multidisciplinary teams
* Ensuring safeguarding, compliance, and statutory responsibilities are met
Essential Requirements
* Qualified Social Worker
* Current Social Work England registration
* Demonstrable experience within Adult Social Care
* Experience in Intermediate Care / Hospital Discharge desirable
* Full UK driving licence (essential)
* Ability to work on a hybrid basis
* Willingness to work Inside IR35
