Pendle View is a well-established practice. We are an innovative and supportive training practice in Brierfield, East Lancashire and within easy reach of motorway links, cities, coast and countryside. We have a population of over 8000 patients from a diverse population. We need an experienced pharmacist to strengthen our amazing clinical team. The candidate will act within their professional boundaries, supporting and working alongside a team in general practice. In this role they will be supported by GP Partners, who will develop, manage and mentor them. The post holder will work as part of a multi-disciplinary team partly in a patient facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice. They will also support the achievement of QOF criteria indicators where practical.
Key responsibilities
* Provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
* Ensure the practice integrates with community and hospital pharmacy to utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.
* Support the integration of general practice with wider healthcare teams to ensure better access to healthcare and to manage general practice workload; offer holistic reviews of complex medications across physical health, mental health and specialist medicines; and increasingly use independent prescribing to add value.
* Lead on medicines optimisation, conduct clinical medication reviews and provide medicines advice to staff and patients.
* Support discharge medicine management, reconcile medicines after hospital discharge, and set up systems to ensure continuity of medicines supply to high-risk groups.
* Manage repeat prescribing policy, review requests, monitor dates for reviews and ensure appropriate monitoring tests where required.
* Provide medicine information to practice staff and patients, answer enquiries and follow up to monitor effects of changes.
* Deliver telephone and patient-facing medicines support (clinics) and manage common/minor/self-limiting ailments within scope of practice.
* Undertake clinical medication reviews and make prescribing recommendations to nurses/GPs.
* Review medicines used to reduce unplanned hospital admissions and implement changes to reduce risk in high-risk groups.
* Identify high-risk patient cohorts through risk stratification and collaborate with team to manage medicines safety.
* Engage in information management and medicines quality improvement through audits and data analysis.
* Provide education and training to the primary healthcare team on therapeutics and medicines optimisation.
* Foster collaborative working relationships within and beyond the practice, including PCN colleagues, community and hospital pharmacy teams.

We are expanding our Clinical team at Pendle View Medical Centre and constantly seek the very best in innovation to help us succeed. Pendle View is an expanding practice in Brierfield, East Lancashire, with a growing list size (just over 8000 patients) and EMIS web systems. We currently have 4 GP partners, run an eConsult system, and have access to ARRS roles including other pharmacists who support clinical work. We encourage staff development and regular team meetings to review clinical areas and share learning. We offer training opportunities and potential for some remote working. Informal visits are welcome.

Person specification
Education, Training and Development (Essential)
* Undergraduate degree in pharmacy and GPhC registration. Suitable prescribing qualification is critical.
* Enrolment in review and appraisal systems; commitment to CPD and mentorship.
Experience (Essential)
* Minimum 2 years’ experience as a pharmacist; experience in general practice settings preferred.
* Knowledge of common acute and long-term conditions seen in general practice; ability to problem-solve and refer to seniors/GPs as needed.
* Ability to follow legal, ethical and professional policies and involve patients in decisions about medicines per NICE guidelines.
Desirable
* Independent prescribing qualification or working towards it; understanding of governance and workplace implementation.
* Engagement with Patient Participation Groups (PPGs) and involvement in practice development.
* Ability to drive quality improvement and plan progress within priorities.
