As a Community Healthcare Support Worker with Harrogate District Foundation Trust, you will be based in one of our four Community Care Teams. The service operates from 7am to 8pm daily, with flexible shift patterns available.

Main duties of the job
Assist in assessing individuals' health and social care needs, including:
* Supporting in the assessment process of health and social care requirements
* Measuring and interpreting physical measurements and samples delegated by the team
* Monitoring patient changes, interpreting their significance, and informing the team of notable changes
* Contributing to planning, delivering, and evaluating nursing care and rehabilitation programmes tailored to patients' needs as per their care plans
