Overview

St Ann's Medical Centre have an exciting opportunity for an up to 9 session (minimum 4 sessions) GP to join our growing practice. We are a large, supportive and fun practice team within a PMS multi-professional training practice offering development opportunities and a focus on education and training within the practice and local area. We have just over 19,000 patients across three sites. The successful applicant will be encouraged to take an active role in leadership and development of the practice and to work as part of a cohesive team including GPs, Business Manager, Advanced Clinical Practitioners, Practice Nurses, Pharmacists, Paramedics, Reception and Administrative staff. Flexibility, compassion and a proactive approach are essential.


Responsibilities

* Deliver high-quality medical care to the entitled population.
* Ensure services are provided in accordance with the GMS/PMS contract.
* Generic prescribing in line with local and national guidance.
* Effective management of long-term conditions.
* Process administration in a timely manner (referrals, repeat prescriptions, blood results, and related tasks).
* On a rotational basis, undertake telephone triage and duty doctor roles.
* Contribute to continuous improvement and quality initiatives within the practice.
* Accept delegated responsibility for specific areas or QOF as appropriate.
* Work collaboratively and share practice workload equitably.
* Attend and contribute to weekly clinical meetings and other practice meetings (e.g., PLT).
* Support partners in achieving strategic aims, including income enhancement and expenditure reduction.


About Us

St Ann's Medical Centre is a large, well-established practice committed to high-quality care. Patient list size is approximately 19,000. We are high QOF achievers with excellent GP Patient Survey results and a CQC rating of Good in all areas. The clinical system is SystmOne. We are active in the local Primary Care Network with Clinical Director and PCN Manager within the team.

Job Responsibilities

Purpose of Job

To deal with a wide range of health care needs in the primary care setting, providing medical services to patients and other duties as delegated by the Practice under Personal Medical Service (PMS) regulations.

Main Duties

* Availability for a variety of duties including surgery consultations, telephone consultations, home visits, signing repeat prescriptions, processing blood results, and related documentation in a timely fashion.
* Make professional, autonomous decisions regarding presenting problems (history taking, examination, investigations) whether self-referred or referred by other health care workers.
* Assess health care needs of patients with undifferentiated or undiagnosed problems; screen for disease risk factors and early signs of illness.
* Develop care plans in consultation with patients in line with current practice protocols; provide counselling and preventative care.
* Admit or discharge patients to/from the practice caseload and refer to other providers as appropriate.
* Record clear, contemporaneous notes; collect data for audits; issue acute and repeat prescriptions; prescribe in line with formulary.
* Provide reports (e.g., DWP, medical certificates, death certificates); supervise juniors/nurse team; provide cover or buddy for absent colleagues.
* Maintain electronic medical records, support QOF data, and uphold Deanery requirements for GP Training Practice status.
* Undertake other duties that are typical for a General Medical Practitioner in primary care.

Other Responsibilities

* Awareness of and compliance with policies on prescribing, confidentiality, data protection, and health and safety.
* Lifelong learning and audit to ensure evidence-based practice; contribute to evaluation, audit and standards setting; ensure Read coding accuracy.
* Participate in training and events; engage in annual performance review and professional development; undertake clinically-related admin duties as needed.


General and Working Conditions

* Annual performance review and adherence to health and safety policies.
* Support risk management; promote equal opportunities; involve patients in local health service decisions where appropriate.
* Discretionary ability to act within policies; maintain confidentiality of patient and practice information.
* Be mindful of QOF targets and contribute to achieving maximum points.

* GMC Registration
* UK GP Qualification


Other

* Disclosure and Barring Service (DBS) check will be required as part of the role.
* Current UK professional registration is required.
