STEPs team (short term enablement programme) needs Community Reablement Officers who can demonstrate the 6 C’s — Care, Compassion, Competence, Communication, Courage and Commitment.
You will support people in their own homes for up to 6 weeks following a hospital stay or to prevent a hospital admission. You will assist and support people to live the way they choose in a safe environment while promoting independence with practical support as agreed in their support plan. This is called reablement.
Responsibilities
* Provide personal care.
* Assist people with mobility transfers following a movement and handling plan.
* Provide support with medication and meals.
* Understand safeguarding and risk assessments.
STEPs are regulated by the Care Quality Commission; you will understand how ways of working in the team make sure we maintain the highest standard of care and excellence.
Qualifications
Essential Driver Post: you will need a full driving licence and own transport to travel throughout the borough.
