What you will do:
* Conduct assessments and develop care plans tailored to the needs of children with disabilities
* Provide dedicated support to children and their families, ensuring their voices are heard and respected
* Collaborate with multi-disciplinary teams to coordinate comprehensive care
* Facilitate best interest meetings in line with the Mental Capacity Act
* Offer advice and guidance on accessing services and resources
