An exciting opportunity has arisen within RiversdaleSurgery for a dedicated and compassionate Practice Nurse to join our friendlyand supportive healthcare team working up to 35 hours per week..
We are a semi-rural vibrant practice based in the heart ofthe market town of Belper, gateway to the Peak District, serving 13,600patients. We have bought together a highperforming multi-disciplinary team including five GP Partners, six SalariedGP's, a nurse team of seven, two frailty ACP's, Clinical Pharmacists, CareCo-ordinator, Mental health workers, FCP service, PCN Home visiting team and afantastic support team.
The ideal candidate will play a vital role in providinghigh quality patient care, assisting with clinical procedures and ensuringsmooth operation of the practice. Thisposition requires a strong commitment to patient wellbeing and the ability towork collaboratively within a multidisciplinary team.
Main duties of the job
* Provide general treatment roomcare including wound management, vaccinations, and chronic disease management.
* Implement and evaluatetreatment plans for chronic disease patients that promote health and wellbeing.
* Identify manage and supportpatients at risk of developing long term conditions.
* Provide routine nursing careto patients in accordance with clinical based evidence, NICE and the NSF.
* Implement vaccinationprogrammes for adults and children.

Job responsibilities
To be responsible for the implementation of processes for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients.
Working as a member of the practice multidisciplinary team, thepost holder will ensure nursing services are delivered effectively to theentitled patient population.
The Practice Nurse will be responsible for a number of clinicalareas such as infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure.
This organisation must ensure that thepost holder has access to appropriate clinical supervision and an appropriatenamed individual in the organisation to provide general advice and support on aday-to-day basis.
