What you will do: Conduct thorough social care assessments, ensuring timely and accurate delivery of care packages for hospital inpatients before their discharge. Perform mental capacity assessments, best interest decisions, and safeguarding measures as required to maintain the highest standards of care. Attend Section 117 meetings, ward rounds, and professional gatherings to offer insights and develop holistic care plans. Support the discharge processes from PICU and acute wards, facilitating smooth transitions for service users. Complete comprehensive Ministry of Justice Reports to uphold legal standards and requirements. Participate actively in the duty rota to ensure consistent service delivery. While not mandatory, having a car and experience with Liquid Logic LAS is preferred, adding to your ability to navigate services efficiently.
