Job summary
To beresponsible for the implementation of processes for the effective management ofpatients with long-term conditions using evidence-based practice including carefor elderly and housebound patients.
Working as a senior member of the practicemultidisciplinary team, the post holder will ensure nursing services aredelivered effectively to the entitled patient population.
The Practice Nurse will be responsible for a numberof clinical areas such as infectionprevention and control, health promotion, chronicdisease management, as well as actively supporting the practice management teamin the reviewing and delivery of clinical policy and procedure.
The Cedars Surgery must ensure that the post holder has access toappropriate clinical supervision and an appropriate named individual in the organisationto provide general advice and support on a day-to-day basis.
Main duties of the job
To work within our already established nursing team alongside several experienced nurses.
We are ideally looking for someone already trained in Asthma, COPD and/or Diabetes.Reviewing these patients alongside the rest of the chronic disease nursing team.
