Job Summary
A Vacancy at West Hertfordshire Teaching Hospitals NHS Trust. The Virtual Hospital at West Herts treats acutely unwell patients remotely, supporting early discharge and improving bed flow.
Key Responsibilities
* Coordinate patient pathways across heart function, asthma, bronchiectasis, COPD, general medicine, frailty and post‑surgical colorectal routes.
* Ensure protocols are followed, track patients, update Cerner records and maintain bed boards.
* Escalate potential issues to clinicians and AHPs, and liaise with the MDT, GPs and other care providers.
* Participate in data collection and link with IT to manage VH systems.
* Document and follow up on administrative actions, protecting the patient journey on the VH pathways.
Working Hours
Shift‑based roster shared by the admin team. Expected shifts of 8am‑6pm across 37.5 hours per week (full time). Roster includes weekends.
