We are pleased to offer the position of Advance Nurse Practitioner. This is a part time position, working 3 days a week. We are looking for an enthusiastic, reliable and friendly individual to join our team.
Main duties of the job
The Advanced Nurse Practitioner role will involve working alongside a multidisciplinary team, including GPs, Clinical Pharmacists and Nursing Team to provide care and treatment to patients with either long term conditions or undiagnosed undifferentiated diagnoses.
The successful Advanced Nurse Practitioner will work as an autonomous practitioner, diagnosing, managing, prescribing and discharging patients with illness. This may be face-to-face consultation or telephone consultation.
Working in a flexible approach and in collaboration with the GPs and other health professionals to provide comprehensive healthcare sensitive to the needs of the local population.
As an independent prescriber, ensure delivery of safe, effective, and appropriate medication in a cost-effective manner, following best practice/national guidance and as defined by current legislative framework.
Promote health and well-being through health promotion, health education, screening and therapeutic communication skills.
Work with multi-disciplinary team within the Practice to promote integrated and seamless pathways of care.

Ystradgynlais Group Practice are offering an exciting opportunity to join our practice based over 3 sites at Pengorof, Ystalyfera and Abercrave.
We currently have a team of 9 Partners, 2 Salaried GPs. we have a multidisciplinary team to include pharmacists, advanced care nurses, practice nurses, HCAs and phlebotomy services. We also have our Cluster Pharmacy Team and Physiotherapists.
We are a GP training practice, and offer many enhanced services. We also have medical and nursing students.
We are an extremely friendly and supportive team, with a patient list size of 12,500.
Job responsibilities
The successful post-holder will deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
The job will include a variety of duties, including surgery/telephone consultations, queries, checking and signing repeat prescriptions, paperwork and correspondance which will all be dealt with in a timely fashion.
