We are a great team providing a high standard of care. All practices are local to each other.

Job responsibilities
* Job Purpose: To improve patients health outcomes and the efficiency of the practice team by providing direct, accessible and timely medicines expertise, including identifying areas for improvement and initiating change.
* To be the primary contact within the practices for queries relating to medicines management, in particular around individual patient care.
* To focus on individual patient care by addressing immediate and future needs through personalised care planning
* Proactively transfer workload relating to medicines optimisation from other clinical staff to improve patient care and safety.
* Demonstrate the effectiveness of the practices and PCN, and the CP role through audit and data analysis
* Take on general practice workload including initial assessment of common presentations, recognise signs of serious illness, differential diagnosis and independent management of minor ailments
* Follow up patients and investigations, particularly blood tests
Job Summary:
* Pharmacist will work within GP practices in the PCN, supported by a senior clinical pharmacist and GP leads; line management by the PCN Manager. They will travel between surgeries to ensure equitable access to services.
* Work within a multidisciplinary team including GPs, nurses, HCAs and administrative staff, with potential collaboration with social prescribing link workers, physiotherapists, paramedics and other roles as nationally proscribed. The Pharmacist will contribute medicines expertise to holistic care.
* Take responsibility for chronic disease management within the practice and undertake clinical medication reviews for patients with complex polypharmacy (focus areas may include CVD, COPD, Asthma, Diabetes, Frailty).
* Provide primary support to staff for prescription and medication queries; support repeat prescriptions, acute requests, medicines reconciliation on transfer of care, and systems for safer prescribing. May run minor ailment clinics as appropriate.
* Provide clinical leadership on medicines optimisation and quality improvement; manage aspects of the quality and outcomes framework and enhanced services.
* Ensure integration with community and hospital pharmacy to optimise skill mix and workload management.

* Clinical - Long Term Conditions / Chronic Disease Management
* Undertake disease-focused medication reviews for high-risk groups to improve effectiveness and safety, ensuring compliance with national/local guidelines.
* Manage patients with single or multiple conditions requiring medicine optimisation (e.g., respiratory, hypertension).
* Ensure safe and appropriate care for patients on high-risk drugs (e.g., DMARDs, anticoagulation); review ongoing need for each medicine; titrate as needed; monitor and arrange recalls; support administration of medicines; provide telephone advice; promote health measures including flu vaccinations; discuss recommendations with Senior Pharmacists or GPs; undertake audits of LTC groups.
* Clinical - Repeat Prescribing and Medication Reviews
* Develop and quality-assure repeat prescribing and medication review processes; conduct structured reviews with patients and create management plans for future prescribing and monitoring; update processes per policy to reduce wastage.
* Carry out clinic-based or home-based reviews and ensure records reflect changes; manage recall dates.
* Clinical - Acute Illness Pharmacist led clinics
* Assess patients with undifferentiated minor illness within scope of practice; initial assessment and referral where appropriate; triage and educate patients on self-care and signposting to relevant services; provide telephone advice for acute illness queries.
* Risk Stratification around medication harm and avoidable hospital admissions
* Review medicines linked to unplanned hospital admissions; implement changes to reduce prescribing in high-risk groups; identify high-risk cohorts via practice data searches.
* Management of medicines at discharge from hospital
* Reconcile medicines post-discharge; ensure continuity of supply for high-risk groups; reconcile records with discharge medications; align with policies; organise reviews and monitoring; inform patients of changes.
* Team Information, Education and Training
* Answer medicine-related queries from GPs and staff; plan training to ensure safe prescribing.
* Implementation of guidelines, safety alerts and formulary recommendations
* Implement changes from MHRA alerts and new guidance; advise on NICE guidance; maintain formulary compliance; monitor prescribing against formulary and advise on red/amber drugs;
* Audit and Monitoring
* Plan prescribing audits, analyse data to inform performance, monitor targets, and implement actions from audits.
