We are seeking a compassionate, innovative, and enthusiastic Band 4 Assistant Practitioner to join our Southern Rapid Response and Intermediate Care team.
This role involves assessing, planning, implementing, monitoring, and evaluating exercise and education programmes tailored to the needs of referred individuals. The role requires working independently and collaboratively within a multidisciplinary team.
Candidates should have experience working with patients with complex healthcare or rehabilitation needs and demonstrate clinical reasoning skills. This enables assessment and treatment of patients within service protocols without immediate supervision.

Main duties of the job
* Deliver clinical and therapeutic care within set protocols and scope of the Assistant Practitioner.
* Perform patient assessments, develop, implement, and evaluate care/treatment plans following competency and delegation from a registered practitioner.
* Manage a caseload of non-complex patients.
* Coordinate safe admission, transfer, and discharge of patients, including discharge planning.
* Assist patients with personal hygiene, grooming, and dressing, respecting cultural and religious needs, ensuring privacy and dignity.
* Ensure safe and competent use of all equipment and mobility aids.
* Report equipment failures or accidents promptly following departmental procedures.

Person specification
Experience
* Experience in a patient-facing environment with a reablement/rehabilitation focus.
* Experience working with patients with physical, cognitive, or behavioral impairments.
Education, Qualifications & Training
* Level 3 Health and Social Care Diploma or equivalent.
* Commitment to undertaking a Foundation Degree and related training when available.
Skills, Abilities & Knowledge
* Knowledge of the audit process.
* Ability to collaborate with multidisciplinary teams and service users.
* Self-motivated with the ability to work independently and in teams.
* Effective communication skills (verbal and written).
* Good organizational skills and awareness of health & safety policies.
* Ability to learn and adapt, undertake manual handling, and work under pressure.
Special Circumstances
* Valid driving license for travel across Buckinghamshire.
