As a PCN Team leader you will provide high-quality leadership and line management to our Care Coordination team and support the smooth running of patient-facing services across our Rushcliffe practices.
About The Role
As PCN Team Leader, youll:
* Lead and manage the Care Coordination team across all Rushcliffe PCN practices
* Oversee the day-to-day operations of patient support services and ensure high standards of care and communication
* Provide operational and leadership support to practices, including acting as a Practice Team Leader when needed
* Support implementation of service changes and manage Enhanced Access provision along side the PCN Service Manager
* Liaise with internal and external stakeholders to improve services, resolve issues, and implement best practices

* Previous team leadership or management experience, ideally in a healthcare setting
* Strong interpersonal and communication skills
* Excellent IT literacy (including Microsoft Office and ideally SystmOne)
* A proactive and flexible approach with the ability to prioritise and self-manage
* Full UK Driving Licence and access to a vehicle
Desirable:
* Management qualification
* Experience working in General Practice or Primary Care
* Knowledge of clinical audits, Quality and Outcomes Framework (QOF), and IIF indicators

Job Summary:
Provide line management to the PCN Care Coordination core team and provide support services to PCN practices to ensure excellent patient care. Act as a General Practice Team Leader when required, providing high quality leadership to practice staff in line with PartnersHealth operational objectives
Job Responsibilities:
* To lead & line manage the PCN Care Coordination core team
* To establish & lead meetings across the PCN to support this: establishing care co/reception/med sec forums
* Provide training on relevant clinical system to the Care Coordination team and wider practice teams where necessary
* To work within practices as & when required to support operational sustainability
* Provide cover as Practice Team Leader when required
* To work across all PCN practices to support implementation of change
* Ensure the smooth running of the Rushcliffe Enhanced Access service on behalf of practices by monitoring and co-ordinating rotas and services to meet the population demand
* Ensure the smooth running of other PCN services such as the Mental Health Practitioner service
Management Of Staff:
To lead & line manage the PCN Core Care Coordination team across the PCN
* Organisation of rotas (appointment booking and annual leave) are implemented to always ensure an adequate and safe level of staff
* Assist in monitoring and evaluating performance of staff against objectives, help to identify and manage change
* Monitor relevant day to day administration, in relation to patient services ensuring it remains effective
* Maintain effective communication both within the Practice and within all the relevant agencies
* Arrange team meetings; prepare agendas and distribution of minutes as necessary
* Assist the PCN Service Manager in the implementation of all local and /or national enhanced services where the Practices are participating
* Assist in the development of protocols, review and update as required in conjunction with the PartnersHealth management team
* Organise and manage meetings to promote good team working across the practice teams
* Liaise closely with all other members of the Primary Care team (internally and externally)
* Responsible for recruitment & selection, supervision, appraisals, organisation of team a/l, first line for sickness, disciplinary etc
Information Technology:
* Act as a subject matter expert of IT/Telephony services within the Practices and ensure that the use of the systems is maximised
* Liaise with NHIS and other IT providers to solve IT problems and ensure a smooth running of practice information systems
Patient Services:
* Always ensure compliance with patient confidentiality, in line with current legislation
* Working with the Practice Team, ensure the relevant patient services are provided and maintained
* Ensure all patient information is accurate, easily accessible and kept up to date
* Participate in regular audits across all services regarding patient satisfaction conducted in conjunction with the reception team and phone system audit
* Be the first point of contact of for complaints and resolve as appropriately, ensuring relevant reporting of complaints and incidents
* Initiate, develop and maintain excellent third-party relationships, e.g.: patient group and other stakeholders to grow, maintain and support services and performance delivery
* To develop understanding of all current PCN services i.e. Enhanced Access, mental health practitioners, spirometry & ensure care coordination support to these teams is optimal
* To ensure admin activities such as rota management, clinical system support and referral processes are maintained along side the Care Coordinator teams
* To develop understanding of IIF indicators & processes required to support monitoring of achievement
* To work alongside the PCN Service Manger to deliver contractual obligations for the Rushcliffe Enhanced Access Service
