Overview
Lead Pharmacist WBC PCN - Hereford City at Taurus Healthcare Limited / Herefordshire General Practice. WBC is a small network of 3 GP practices in Hereford City with an established PCN Pharmacy team. We are seeking an experienced Pharmacist to lead our team.
Responsibilities
* Provide prescribing advice and support to practice clinicians, staff and patients, including handling prescription and medication queries, medicines policies and procedures, QOF and local incentive schemes, audit and medicines safety.
* Conduct medication reviews, minor illness consultations and reviews post-discharge as part of clinical work at individual practices.
* Lead and mentor a team of pharmacists and pharmacy technicians with regular clinical supervision and development; act as a non-medical prescriber with CPPE National Pathways and supervisor courses completed.
* Work within clinical competencies as part of a multi-disciplinary team to provide senior leadership in medicines optimisation and quality improvement across the PCN.
* Contribute to: health in care homes, management of long-term conditions, medicines on transfer of care, safer prescribing systems, national/local contracts (e.g. IIF, QOF, CEIF, QI), clinical audit, repeat prescription processes, acute prescription handling, and addressing public health and social care needs of patients.

Requirements / Experience
* Minimum of 3 years post-qualification experience.
* Ability to communicate complex information clearly to diverse audiences.
* In-depth therapeutic and clinical knowledge, with evidence-based prescribing principles for primary care, especially long-term conditions.
* Strong IT skills (spreadsheets, databases, reporting).
* Ability to influence and motivate others; empathic and reassuring communication style.
* Understanding of prescribing best practices and legal requirements; ability to manage a team and provide clinical support and mentorship.
Qualifications
* Holds, or is working towards, an independent prescribing qualification.
* Completed CPPE National Pathways and related supervisor training; evidence of CPD and lifelong learning for revalidation.
* GPhC-registered Pharmacist (MPharm).
Desirable
* Post-graduate diploma or equivalent advanced knowledge; CPPE national training pathways supervisor training.
* Membership: Primary Care Pharmacy Association (PCPA) and Royal Pharmaceutical Society.
Other requirements
* DBS clearance in line with policy; adaptable; able to work independently and as part of a team; safeguarding and mandatory training; immunisation status; access to transport and willingness to travel regionally, including home visits.

Taurus Healthcare Limited, Whitecross Road, Hereford, HR4 0DG. Herefordshire General Practice is the partner network. Pay: £55,188.02 to £59,516.49 per year (pro rata, based on experience). Contract: Permanent. Working pattern: Full-time, Part-time, Flexible.
Reporting and team structure: Supervision by GP and PCN Prescribing Lead; collaboration with pharmacists and technicians within the PCN; contacts for discussion and informal visits via the WBC Lead Pharmacist.
