We are looking for anenthusiastic, experienced Nurse Practitioner to join our friendly clinical teamto help us maintain the high standards we provide to our patients.

Main duties of the job
The successful candidate will beable to:
- Deliver a high standard ofpatient care as a Nurse Practitioner in General Practice, using advancedautonomous clinical skills and must have a broad and in depth theoreticalknowledge base
- Assess patients presentingwith acute illness and exacerbations of long term conditions
- Provide a first point ofcontact within the practice for patients presenting with undifferentiated,undiagnosed problems, making use of skills in history taking, physicalexamination, problem solving and clinical decision making to establish adiagnosis and Management plan

Job responsibilities
The post holder will be responsiblefor patients requesting urgent, on the dayappointments with undifferentiated, undiagnosed illness. Thepost holder must be able to take history, assess, examine, make diagnosis andprescribe medication and treatment post holder must beable to order tests, make referrals as appropriate, and support other membersof the medical team as required. There is a requirement to see extra oremergency patients. The post holder must be able to manage own workload andprioritise appropriately. The post holder will be able to triage patientsappropriately by telephone as required. Work effectively with all of thenursing team and other nurse practitioners. The post holder will oversee thepractices progress towards the QoF targets.
Togetherwith the other Nurse Practitioners become an active and integral part of thenursing service.
Tohelp develop and maintain practice policies and protocols.
Ensurethe practice manager is informed of any matters arising or problems/potentialproblems.
Tohelp work towards the QOF targets ensuring that the highest level of points areachieved. This will involve working closely with all members of the primaryhealth care team.
Tosee patients with undiagnosed, undifferentiated illness.
Ensureclinical practice is safe and effective and remains within boundaries ofcompetence and to acknowledge limitations.
Advisepatients on general health care and management of current illness with referralto other members of the primary and secondary health care team as necessary.
Prescribetreatment, order tests and make referrals.
Providingassessment, screening treatment services and health education advice.
Thisis not an exhaustive list and other duties may be required within scope ofpractice.
Participate in the administrative and professionalresponsibilities of the practice team.
Ensure accurateand legible notes of all consultations and treatments are recorded in thepatients notes.
Ensure theclinical computer system is kept up to date with accurate details recorded andamended.
Ensure thatproactive and opportunistic steps are taken in order to ensure that allpatients are assessed for the QoF targets and that all nursing team is aware ofthese targets (this should be completed on an ongoing basis as targets dochange).
Ensureappropriate data entry is undertaken to ensure the maximum QOF points can beachieved.
Ensure accuratecompletion of all necessary documentation associated with patient health careand registration with the practice.
Ensurecollection and maintenance of statistical information required for regular andad hoc reports and audit.
Taking a leadon areas of interest.
Trainingrequirements will be monitored by yearly appraisal and will be in accordancewith practice requirements. Personal development will be encouraged andsupported by the Practice. It is the individuals responsibility to remain upto date with recent developments.
Together withthe other nurse practitioners develop and maintain an in-house nurse-trainingprogramme reporting to the practice manager.
Maintaincontinued education by attendance at courses and study days as deemed useful ornecessary for professional development as appropriate to the role undertakenwithin the practice and with prior agreement from the practice manager.

Develop andmaintain a personal learning or reflective plan
Clinicalsupervision.
To promote aworking environment with the support of the partners that supports learning andshared values for patient care.
To provideclinical mentorship to the practice nursing team.
To develop andmaintain practice policies and protocols for all nursing duties.
To liaise withthe infection control nurse in the practice to ensure all staff are aware ofcurrent procedures.
Ensure that thepractice manager is informed of any matters arising or problems/potentialproblems.
Together withthe other nurse practitioners organise nurse meetings
Person Specification
Experience
Essential
1. Minimum 1 years working as NP in General Practice.
2. Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of the Nurse Practitioner role.
3. Evidence of working autonomously and as part of a team.
4. Proven ability to evaluate the safety and effectiveness of their own clinical practice.
Desirable
5. Experience in Chronic Disease management of long term conditions such as asthma, diabetes
Qualifications
Essential
6. Registered General Nurse.
7. Recognised Nurse Practitioner qualification at minimum degree level or equivalent.
8. Independent Nurse Prescriber.
Desirable
9. Certificate in Chronic Disease.
