Adults Social Worker – Sensory, Physical & Neurological Team
Contract: Initial 6-month agency contract
Team: Adult Social Care Specialist Teams
We are currently seeking an Agency Social Worker to join our Sensory, Physical and Neurological Team within the Adult Social Care Specialist Teams. This is an initial 6-month contract, with the potential for extension.
You will be part of a supportive and collaborative team that includes a Team Manager, Senior Practitioner, Experienced Social Workers, Social Workers (including ASYE), Community Care Workers, and administrative support.
The team primarily works with people under the age of 65 who have a diverse range of complex needs, including:
* Physical Disabilities
* Mental Health needs
* Learning Disabilities
* Acquired Brain Injuries
* Autistic Spectrum Conditions

Key Responsibilities
* Undertake personalised assessments, support planning, and reviews.
* Work within key legislation including the Care Act, Mental Capacity Act, and Mental Health Act.
* Apply a strength-based approach to practice, ensuring people’s needs, rights, and aspirations remain central to your work.
* Collaborate effectively with individuals, families, carers, and partner agencies to achieve holistic outcomes.
* Manage a caseload with the support of a strong and experienced team.
About You
We are looking for a motivated, dedicated, and resilient Social Worker with a can-do attitude. You will bring:
* A professional social work qualification and current registration with Social Work England.
* Experience of working with adults with complex needs.
* Strong knowledge of safeguarding and relevant legislation.
* Excellent communication, assessment, and planning skills.
* The ability to work collaboratively as part of a multidisciplinary team.
