Integrated Community Coordination Teams – While working in the ICCTs, you will complete strengths-based Care Act assessments and person-centred support planning with a diverse range of service users aged 18 and over. The work aims to promote the wellbeing of adults with care and support needs, continuously improving their outcomes and creating lasting community connections.
Essential criteria:
* Degree in Social Work
* SWE Registration
* Right to work within the UK
Must have:
* A professional qualification in social work
* Social Work England registration
* Enhanced DBS/Update service
* References for the last 5 years of employment
