Job Summary
An exciting opportunity has arisen for the role of Patient Flow Co‑ordinator in the Clinical Site Team.
Cover 7‑day rota: days, evenings, weekends, bank holidays on a rotational basis. Cross‑site working also required.
The purpose: to ensure patients have a seamless journey and discharge, effectively managing the pathway across sites.
Motivated, enthusiastic individual to join dynamic team, support Ward managers and Matrons, support patient pathways from admission to discharge/transfer.
Caring and compassionate person who wants to make a difference to our complex patient group.
Responsibilities
The flow coordinator liaises with the Emergency Department, Clinical Site Team and other Flow Co‑ordinators and receiving areas to ensure effective communication regarding capacity is maintained and appropriate early escalation is facilitated.
Work across both sites, Doncaster and Bassetlaw Sites.
Monday to Sunday – working hours 07:30‑20:00.
Qualifications & Training
* Knowledge of personal care and related procedures
* Basic maths and literacy
* GCSE Grade C in Maths and English or equivalent
* Vocational qualification at level 2 or equivalent relevant experience (desirable)
Knowledge & Experience
* Experience working with people using customer care skills
* Knowledge and understanding of current healthcare issues (desirable)
* Experience working in a healthcare setting and/or providing personal care (desirable)
* Basic computer skills (desirable)

* Good communication skills, work within multidisciplinary team
* Well‑motivated, genuine desire to care for people.
* Good written skills
* Ability to work within a team; support and assist in safe and effective delivery of personal care.
